From Referral-Dependent to a Predictable Lead Engine

Brief

A private medical aesthetics clinic in the UK was running almost entirely on word-of-mouth. We built the paid acquisition and automation infrastructure to turn an unpredictable trickle of referrals into a consistent, measurable weekly booking flow.

Background

The practice had a strong reputation and excellent clinical outcomes, but no reliable way to generate demand. Good months came from a burst of referrals; quiet months had no lever to pull. Enquiries that did arrive were handled manually, so follow-up depended on whoever had time that day.

Aesthetics is a trust purchase, so we kept the creative clinical rather than salesy: real results, clear pricing signals, and a booking experience that feels like a medical practice, not a discount offer.

No lever to pull

The Challenge

Referral-led growth has a hard ceiling. It's unpredictable month to month, it can't be scaled on demand, and it gives you no data. When bookings dip, there's nothing to diagnose and nothing to adjust.

The clinic needed a second, controllable channel: a system where spend goes in one end and qualified consultations come out the other, with every step measured. It also needed to stop losing warm enquiries to slow manual follow-up, which was quietly costing more than the missing ad spend.

Landing page, creative, and tracking

Building the Acquisition Layer

We started with a purpose-built landing page rather than sending traffic to the existing site. Cold paid traffic converts on focus: one treatment, one promise, visible proof, and a single obvious next step. We structured it around the objections that actually stall an aesthetics booking: is it safe, who's performing it, what does it cost, and what happens at the first appointment.

Alongside it we produced a set of Meta ad creatives built for testing rather than a single hero ad, varying the hook, the proof, and the offer framing so the account could learn quickly which angle earned attention at the lowest cost.

Measurement came before scale. We installed the Meta Pixel and the Conversion API together, so conversions are reported server-side as well as in-browser. That matters more than it sounds: without it, iOS opt-outs and browser tracking restrictions hide a large share of conversions, the algorithm optimises on incomplete data, and reported cost per lead looks worse than reality.

Removing the manual follow-up gap

Automation & Nurture

Every lead now lands in a GoHighLevel workflow that responds immediately. SMS goes first, because response time is the single biggest determinant of whether an aesthetics enquiry converts, then email for the detail that needs more room.

From there the sequence nurtures rather than nags: treatment education, practitioner credentials, and clear answers to the questions people are too polite to ask, spaced so the clinic stays present without becoming noise. Prospects who go quiet get re-engaged automatically instead of being forgotten in an inbox.

Booking runs through an automated appointment workflow with confirmations and reminders built in, which cut both the admin load on reception and the no-show rate that eats into a consultation calendar.

A system, not a spike

Outcome

The clinic moved from unpredictable referral flow to consistent weekly appointment bookings, with a cost per acquisition it can actually forecast against. Just as importantly, follow-up stopped depending on staff availability: the automation handles first response, nurture, and reminders, so the team spends its time on consultations rather than chasing enquiries.
